1- Valid and signed Passport
The passport expiration date must be greater than six (6) months (from the date of entry in Brazil).
Passport must have a minimum of two (2) complete blank visa pages to stamp the visa , remember the last three (3) pages are not visa pages and it will not be accepted, if your Passport does not meet the requirements, click here for Passport expedite service prior to obtaining the visa.
Note 1A:If you ever had a Brazilian passport and/or Brazilian birth certificate issued by any Brazilian official and/or not official authority in or out of Brazil in any Brazilian Consulate Embassy any where in the world, in order to get your visa you must prove that you lost and/or give up your Brazilian citizenship (using only the U.S. naturalization certificate may not be enough. 2- One (1) Brazil Visa Application Form
The Visa Application Form must be completely filled-out both pages and signed.

Note 2A: In case that you’re not sure if you have filled out the visa application correctly, please send a second application in blank and just signed. We will help you to fill out. Better work safe then delay the process (but don’t forget you must send one of the forms completely filled and signed) we strongly recommend you to send the second form
Note 2B: Please make sure that you sign this visa form with signature identical as possible to the signature that you have on your passport (Consulate/Embassy may refuse and or delay to give any visa for those who signatures doesn’t match). In case of name change please send a clean and certificated copy of the official papers
3- Photograph
You must provide two (2) front face and full view recent (2” x 2”) photo U.S. Passport type, Homemade Digital Photographs will may not be accepted by the Consulate.

4- Proof of Jurisdiction/Residence
Please send an understandable and visible copy of your Driver’s License.
Note 4A: Consulate will not Accept Driver’s License copy by Fax.
5-Proof of travel
Please send a legible copy of your: itinerary reservation, electronic ticket or round-trip ticket.
Note 5A: Remember your First entry into Brazil must be within 90 days of visa issuance date, after you get the visa you MUST make your first travel to Brazil 90 days if you don't travel to Brazil 90 days of visa issuance date your visa will be automatic declaine and you will need to issue a new one in order to travel to Brazil.
6-Vaccination Information
Applicants who have visited any of the following countries within 90 days prior to the trip are required to present a good copy of valid International Yellow Fever vaccination certificate:
Peru, Bolivia, Colombia, Ecuador, French Guyana, Venezuela, Angola, Benin, Burkina Faso, Cameroon, Congo, Gabon, Gambia, Ghana, Guinea Bissau, Liberia, Nigeria, Sierra Leone, Sudan.
Note 6A: The Brazilian Government Strongly Encourage all travelers of all ages since six (6) months of age to get an anti-Yellow Fever vaccination prior to departure to Brazil for they own safe and protection.
7- Only for Minors:
Minors (under 18 years of age) need notarized copy of birth certificate and parents letter of authorization to travel to Brazil; signed and notarized by both parents and/or guardians (signatures must be notarized by the public notary) (in case of guardians please send a notarized copy or original of the guardian release court papers where it must say that the guardian do have authority to travel with and/or sent this/those minor(s) to travel out of United States).
Note 7A: For Minors (under 18 years of age) of Brazilian citizens, please provide a notarized letter stating that the child has never been registered as a Brazilian citizen in any Brazilian official and/or not official authority in or out of Brazil in any Brazilian Consulate or Embassy in any where in the world, nor has had a Brazilian passport previously. Please, note that Brazilian citizens must travel to Brazil with a Brazilian passport. (Brazilian bearing a American Passport that has renounced the Brazilian citizenship must present proof). Note 7B: Please do not sent any visa application for minors before calling us ph: 786-333-9176, Consulate is very strictly and delicate with visa application for minors so that means minors visa application may delay a bit.
Note 7C: For children under 7 years of age, a copy of International certificate of vaccination against POLIO is required.
8- Only for Non US Passport Holders:
Good and both sides Copy of the Green Card, work permit and/or Copy of any Other Document that proves Legal Status in This Country.
Note 8A: Remember all original documentation that you may send to us we will return for your with you passport.
9- Consulate Tourist Visa Fee:
Consulate fees for U.S. passport holders $140.00 in US Postal money order only payable to the Consulate of Brazil.
10- Our Expedite Tourist Visa Service Fee (Please choose from the follow):
STANDARD Service Fee - $29.00 (10-15 Business Days processing time - not included back and forward courier time from client)
RUSH Service Fee - $49.00 (6-9 Business Days processing time - not included back and forward courier time from client).
URGENT Service Fee - $69.00 (2-5 Business Days processing time - not included back and forward courier time from client).
Note 10A: Business Days to process the application/s means application/s on our hands before 9:00 AM eastern standard time.
Note 10B: applications delivered to our hands after 9:00 AM eastern standard time will not be processed until the next Business Day.
Note 10C: applications send to us as UPS early A.M. or FedEx First Overnight may arrive before office hours in order to don’t loose the delivery we strongly recommend you to choose at the package on (no signature required) and please write with a black mark around the package (please slide the package under the door in case no buddy to sign )
Note 10D: Consulate and/or Embassy may change time - requirements and/or rates with out previous notice.

DHL Second Day Service Shipping and Handling Fee $18.00 (no island, rural and/or remote locations, for those price may change, please call for accuracy availability price and time).

Yes you can provide you own return envelop and prepaid air bill from (DHL / FedEx / UPS) plus $5.00 for return handling fee. (Obs.: please use the right label if is possible the online ones and don’t forget to keep one for you self in order to track the package back to you)
Note 11A: We do not accept any return envelop from any courier with C.O.D. (Collect On Delivery) Label.
Note 11B: We do not have any supply at all from UPS (United Parcel Service) so in case you choose to have your passport return by UPS please be advised to provide your own and pre paid complete (label and package) return supply.
Note 11C: our return shipping options & fees are based on 8oz letter size envelop and/or 4 passports.
Note 11D: We do not check the Postal Mail on a daily basis so if you are sending your passport by USPS (United States Postal Service) with (delivery confirmation and/or signature requested) or not, there is a good chance that your application may get delayed.
Note 11E: Sorry we DO NOT return passport(s) and/or any docs by USPS (United States Postal Service).
Note 11F: All of our shipping returns are included (Signature Required on Delivery) except if this service is not standard-offered by the courier company, WE STRONGLY ENCOURAGE you to do the same.
Note 11G: FedEx and DHL no longer require a signature on deliveries to residential address. Unless you feel comfortable with this policy, WE STRONGLY ENCOURAGE you to use your business address for return shipping.
12- Payment method:
We only accept payments by US Postal Money Order and/or Credit Cards Amex/Discovery/Master-Card/Visa.
Note 12A: The Consulate visa fee must be paid by a US Postal Money Order only and payable to Consulate of Brazil the consulate will not accept any other payment besides US Postal Money Order.
Note 12B: Yes you can write the total fees together including the Consulate visa fee on only one: U.S. Postal Money Order, or Credit Card payable to: SOUTHRIVER INTL INC. We will split the payment on the way needed to process client’s visa service under the client’s service choice from this page.
Note 12C: We do not accept any payment provide by the following: Personal Check, Company Check, Cashier Check and Non US Postal Money Order.
Note 12D: All Credit Card payments are subject to a $4.50 or 4.50% fee whichever is greater on the top of the total amount for merchant processing charges. (Obs.: Credit card must belong to the visa applicant otherwise please send a clean copy the credit card holder driver’s license for signature identification)

Note 13A: This form is very important in order to well represent the client, it give to us the official authority to apply answer and retrieve client’s passport and/or paper work before any Consulate/Embassy of Brazil in United States.
Note 13B: This form is one of the new requests made by some Consulate authorities and they may not issue client visa with out this form, please full read print fill and sign this form and send to us with the other papers. (Obs.: In of this is a visa application for a minor (under 18 years of age) please both parents must notarize signatures, please follow the form indications.)
