Requirements for US Citizens

1. US valid/signed passport in good condition with at least two blank visa pages (Note: pages 22, 23, 24 are NOT visa pages)
2. Two original visa application forms signed, completely filled out informing purpose of the trip
3. Two recent 2in X 2in passport type photos
4. Proof of financial ability to pay for one's stay in Brazil
5. Good conduct record from local police department (for over 90 days of stay)
6. A copy of the school registration or confirmation of acceptance from the Brazilian educational institution
7. A copy of a driver's license
8. Minors (under 18 yeas of age) Notarized parent's authorization letter for a child to travel to and from Brazil, addresses/names of responsible parties in Brazil
9. Consulate fee $150.00 US Postal money order only (You can obtain at the Post Office) payable to the Consulate of Brazil
10. Master Visa Service fee $55.00 payable by Money order to Master Visa Service
11. Pre-paid envelope to return the passport with the visa to you
